HUB Insurance -> Questionnaire

Hi everyone,



I recently received a letter from HUB Insurance in regards to the AVIVA Insurance.

Basically they are asking me to fill out a questionnaire of basically all kinds of things in regards to the property: when was roof was change, is it vacant, oil tank, plumbing, electrical, sewer backup, etc



It only says that it is required for this year only.



Has anyone got anything like that?
